Maine is a “Direct Access” state, meaning Maine residents may seek physical therapy care without a referral. Services must be provided by a physical therapist licensed to practice in the State of Maine and adhere to the following guidelines:
Physical Therapists are not permitted to make a medical diagnosis.
If no improvement is documented within 30 days of the start of treatment, or treatment extends beyond 120 days, the client must be referred to a licensed doctor of medicine, osteopathy, podiatry, dentistry or chiropractic.
Residents of any state may schedule wellness visits.

Decades Physical Therapy collects payment from clients at the beginning of each appointment using a secure payment portal.

A Good Faith Estimate (GFE) is required by law for clients who are uninsured or choosing to pay cash for physical therapy services. After specific concerns have been reviewed, a verbal quote will be discussed, followed by a more detailed, written GFE that including treatment codes and the estimated number of visits/cost.

Because Dr. Cowan is licensed to practice in the State of Maine, physical therapy clients must be Maine residents and located within the state during their visit.
Wellness clients are free to reside and attend visits from anywhere.

Life can be unpredictable, so a 10 minute grace period is extended for all visits. If you have not logged in to join during that time, the appointment will be considered unattended.
Advanced notice of 48 hours is kindly requested to reschedule or cancel. Additional fees may be incurred for appointments rescheduled or canceled with less than 48 hours notice.
Unattended visits will be charged the full fee of the scheduled visit type.

At this time, Decades Physical Therapy is a telehealth only practice. A screening phone call or email communication will help to determine if your concerns can be managed in this format. If online care is not reasonably expected to meet your needs, Dr. Cowan will make an alternative recommendation.

Insurance

“Out-of-Network” only applies to Physical Therapy visits.
This means that Decades Physical Therapy has no contract with any health insurance company. Payment is collected at the beginning of each visit and a detailed bill is provided so that you may request reimbursement from you insurance provider.
Reimbursement is not guaranteed.

Clinics that bill insurances typically charge $400+ for an initial consultation, and $200+ for a 25 minute follow up visit.
Cash based practices typically charge less than half of that offering great value - especially for those with high insurance copays or deductibles.

Wellness visits are not reimbursable by insurance, which means appointments are available to anyone regardless of their insurance plan or state residence.
Unfortunately, private payment for medically necessary Physical Therapy Services is not currently permitted by Mainecare, Medicare, Medicare Advantage plans.

Telehealth

Decades Physical Therapy uses the Jane APP secure platform for telehealth and medical records. You will not need to download any software or create additional accounts to participate in telehealth. Appointments can be joined securely via email, text message link, or the Jane Mobile App.
You will also have access to a secure client portal that is set up through a welcome email or when booking their first visit online. The portal provides access to receipts, education materials, and other helpful documents.

Device (smartphone, tablet, laptop, computer): Your device must have a working webcam/camera, microphone, and a stable internet connection.
Supported browsers include: Google Chrome: Version 98 or newer - Mozilla Firefox: Version 101 or newer - Apple Safari: Version 15 or newer - Microsoft Edge:Version 116 or newer.
Connection Speed: At least 15-25 Mbps download and 5 Mbps upload for optimal video and audio quality. You can test current speeds online using fast.com
Low Data Mode: For weaker network connections, toggle on "Low Data Mode" option within the Jane App Telehealth Settings to reduce video resolution and prioritize audio.
